Omair Siraj - Chief Executive Officer

Omair Siraj is the Chief Executive Officer  and Founder of Chestnut Health. Omair's vision for providing high quality care to the acute and post-acute world is what drives the success of our practice. He's had tremendous success in several healthcare and business ventures throughout his career. 

Chestnut is a group medical practice that took birth as a consequence of the emerging needs in the health care industry due to the Affordable Care Act (ACA) 2011. It specializes in providing customized clinical programs through its physicians and physician extenders to  hospital systems, LTACs and sub-acute rehabilitation facility networks (SNFs). It primarily aims to enhance the quality of patient care and clinical outcomes, while mitigating the overall cost of care. Prior to launching Chestnut Health, he served as the CEO of Hospital Medicine Company (HMC), a hospitalist company serving several major hospital systems in Chicagoland. In 2015, he successfully completed the acquisition of HMC by IPC The Hospitalist Company, Inc., (NASDAQ: IPCM), a publicly traded national company with over $1.4 Billion market value. 

Jason Graf, PA-C is a Certified Physician Assistant. His specialty is Physical Medicine and Rehabilitation with more than 16 years of experience. Jason graduated with honors from Midwestern University with a Bachelor of Science and a Master of Medical Science. Jason has a proven track record of the highest level of care and patient satisfaction. 

Jason is published and is also passionate about education. He’s been invited to present at numerous events including “Rotator Cuff Diagnosis and Treatment”, “Anatomy of Upper and Lower Extremity” and “The Role of the Physician Assistant in the Field of Orthopedics”. With his background he takes a keen interest in athletic training. This has helped Jason treat patients during recovery with an understanding of how to return to the highest functioning level possible. 

As Chief of Clinical Operations he brings his understanding of direct patient care to the broader vision of Chestnut Health. Jason provides management, vision, organization, and strategies to achieve a continuum of care that improves the lives of our patients. Jason is responsible for the day-to-day operations of the company both from a clinical and managerial prospective. His emphasis is on clinical competence, compliance, training of new office staff, and training of new providers.
